Services We Provide

  • Monthly Home visits and telehealth visits
  • Maternal and infant health and wellness monitoring
  • Education on your specific risks, including weight management, diabetes, hypertension in pregnancy and after pregnancy
  • Labor and delivery preparation visit
  • Nutrition education
  • Home visit, support and education after your delivery
  • Newborn home visit
  • Infant growth and development monitoring
  • Safe sleep and infant safety education
  • Mental health screening, education and referrals
  • Social needs screening, education, and referrals
  • Assistance identifying a medical home for mother and baby
  • Care coordination with healthcare providers
  • Referrals for Medical insurance, WIC, SNAP, immunizations, transportation, housing, childcare, employment services, family planning, behavioral health services, and other community resources

Who is Eligible?

You can qualify for our services if you:

  • Are currently pregnant,
  • or have given birth within the last 6 months,
  • or have an infant up to 18 months of age.

And:

  • You are at risk for one or more maternal, infant, medical, behavioral health, or social reasons below,
  • and you live or receive health services in one of the following counties: Panola, Alcorn, Oktibbeha, Neshoba, Lauderdale, Adams, Pike or George.

At-Risk Conditions

The lists below include some, but not all, of the at-risk conditions that can qualify for Healthy Moms, Healthy Babies services. Contact the HM/HB program to find out whether your particular situation qualifies you.

Mothers: Mothers may qualify if conditions exist that put their pregnancy or the health of their baby at risk. These conditions include:

  • Having a chronic illness such as diabetes or hypertension
  • Tobacco, alcohol or substance abuse
  • Living in an unsafe environment, such as inadequate housing or being a victim or violence or abuse
  • Being 19 or younger and pregnant

Infants: Infants may also qualify if they have certain medical conditions. These include:

  • Being born prior to 37 weeks of gestation
  • Weighing less than 5 lbs, 8 ozs at birth
  • Other medical or psychosocial conditions affecting your infant's health or development.

Our risk screening will look for these and other conditions that can qualify you or your baby.

What is Your Role?

This is a voluntary program. If you decide to participate, you will be asked to:

  • Participate in your plan of care and attend appointments with the HM/HB Care Management Support Team.
  • Provide a working contact number for your care management team to reach you.
  • Keep your care management team up to date about appointments with other healthcare providers.
  • Let the care management team know if you are having any problems with your pregnancy or baby.
  • Ask questions to help us better serve you.
