Chronic Disease Self-Management Program
The MSDH Office of Preventive health offers to the public the Stanford University Chronic Disease Self-Management program and the Diabetes Self-Management Program — a Free six-week step-by-step workshops for adults who are living with or caring for someone with a chronic condition or disability.
You don't have to stop doing all the things you love in order to enjoy a healthier life!

By taking part in Motivated to Live a Better Life you will learn ways to cope and manage your health by:
Participants meet 2½ hours each week for six weeks. Workshops are held in community settings such as churches, community centers, libraries, hospitals and other locations throughout the state. There is no charge to attend a workshop.
