Under Mississippi's Life Equality Act of 2020 (§ 41-41-401), "in every case in which a physician performs or induces an abortion on an unborn human being, the physician shall within fifteen (15) days of the procedure cause to be filed" a written report with the Office of Vital Records, on the form below, a report of the induced termination of pregnancy performed in Mississippi.
