We can provide a text listing of licensed professionals to you electronically by e-mail for a $50 fee.

Format

The lists are alphabetically ordered unless otherwise specified. Please provide all information desired.

• Home telephone number and social security numbers cannot be released.
• The data is in ASCII format.

Requests and Payment

A written request specifying the desired information and a check or money order in the appropriate amount made payable to the Mississippi State Department of Health is needed to fulfill the request. Assistants are included in each profession only if they are specified in your original written request. Please allow approximately one business week for our office to process your request.
